Teledoc - Virtual Cardiometabolic Program

Team members and their dependents enrolled on the Anthem plan have access to the Teladoc virtual
cardiometabolic program. More details can be found at www.teladochealth.com.

For those facing a cardiometabolic-related diagnosis, Teladoc can help! This program is available at no cost for those enrolled in an Anthem medical plan. Participants are provided tools to help monitor their condition and coaches to provide guidance and support through the journey.

Diabetes Management: Available to those with a type I or type II diagnosis. Upon registration, participants receive a Welcome Kit, including a blood glucose meter, a lancing device, and a carrying case. Once the meter is set up within a participant’s secure Teladoc account, they may access online resources, participate in challenges, and discover new healthy habits for managing their diabetes. Teladoc will provide the necessary support when readings fall outside the optimal range and provide expert coaching to guide participants through the journey.

Hypertension Management: Upon registration,
participants will receive a Welcome Kit, including
a blood pressure monitor. Once the monitor is set
up within a participant’s secure Teladoc account,
they may access online resources, participate in
challenges, and discover new healthy habits for
managing their hypertension. Teladoc will provide
the necessary support when readings fall outside
the optimal range and provide expert coaching to
guide participants through the journey.

Weight Management: Participants with a BMI
greater than 25 can enroll and gain access to
personalized tools and support to sustain health
weight loss. Participants receive guidance on
building healthy habits around nutrition, activity and
sleep, along with personalized one-on-one coaching
to set goals, navigate challenges and stay on track.

Diabetes Prevention: Available to members who
may be at risk of type II diabetes. Upon registration,
you will receive a Welcome Kit, including a smart
scale to sync with Teladoc’s web portal. Participants
also have access to expert coaches to assist with
nutrition, healthy diets, and staying active.
