​2027 Wellness Program and 2028 Wellness Incentive
Eligible team members who participate in the wellness program and meet the requirements of the wellness
incentive will receive a $50 per month discount off their medical premiums the following year. To achieve the
discount, you must accomplish the steps below.

Premium Discount

Six Flags offers team members the chance to earn rewards by participating in key activities. Those who meet the requirements in 2026 will enjoy a $600 annual discount on medical coverage in 2027.

Step 1: Complete a biometric screening
by the deadline
Download the Physician’s Results Form from the Wellness
Locker in the Wellness portal and take it to your medical
provider during your annual preventive exam. After the
exam and screening, your Primary Care Physician can
complete the form. It is your responsibility to collect the
completed form and submit it to the Wellness portal
(www.wellworksforyoulogin.com).

Please note: It is the participant’s responsibility to
ensure necessary forms have been submitted by the
deadline. It is advised to reach out to Wellworks for You
before the deadline to confirm you have met the steps to
obtain the incentive.

Step 2: Complete the online assessment from
Wellworks for You
You can access the Know Your Number (KYN)
Assessment from the menu page after logging in to the
Wellness portal.
 

Be sure to access the Wellness Portal
(www.wellworksforyoulogin.com) with
any questions you may have.

Be sure to complete both steps before the deadline
of October 31, 2027, to secure the wellness incentive
for 2028!

Team members who are unable to meet the
requirements for this wellness program may qualify
for an alternate opportunity that would allow them to
qualify for the wellness incentive. Contact Wellworks for
